Intake and consent forms

1. Intake and consent forms

   Questions and consent a customer completes after booking, per service. Answers are encrypted. This tour walks through the page.

2. New form

   Opens the form editor: title, introduction, consent text, signature, which services use it, and the questions with their type.

3. Your forms

   Each form shows its version and how many questions, services and responses it has. Edit it, translate it, or delete it. A form with responses is switched off and kept instead.

4. Forms still open

   Upcoming appointments where the customer has not completed a form yet.

5. Responses

   Submitted forms with the customer and appointment. Opening a response is recorded in the audit log.
